The first paragraph is your introduction . Indicate what you are seeking representation for (acting, dancing, modeling, etc.)  Be nice but brief and to the point.  If you know someone who is listed with the agent and has referred you , you may use their names.

I have enclosed my headshot/resume/self-addressed-stamped-envelope and I would appreciate an opportunity to meet with you personally to discuss the possibility of listing with your agency. My schedule is flexible, so I can make my self available at a time that is convenient for you.
